Tramadol-related deaths have occurred in patients with previous history of emotional disturbances, suicidal ideation or attempts as well as history of misuse of tranquilizers, alcohol, and other CNS-active drugs. The use of this agent it not recommended in patients who are suicidal or addiction-prone. Patients should be monitored appropriately and observed closely for worsening of their symptoms, suicidality, or changes in their behavior, especially during the first few months of treatment, and at times of dose changes. Patients, caregivers, and families should be alert to the emergence or worsening of signs and symptoms of depression, any unusual changes in mood or behavior, or the emergence of suicidal thoughts or behavior. For clinically significant or persistent symptoms, a dosage reduction or treatment withdrawal should be considered. If patients have symptoms of suicidal ideation or behavior, treatment should be discontinued.
