The manufacturer considers the use of metaxalone to be contraindicated in patients with severely impaired hepatic and/or renal function. Metaxalone is metabolized by the liver and subsequently eliminated by the kidney, thus both parent drug and metabolites may accumulate in such patients. Additionally, metaxalone may be hepatotoxic. Jaundice as well as abnormalities in liver function tests, including elevations in serum AST, ALT, alkaline phosphatase and bilirubin, have been reported. Nephrotoxicity and proteinuria have also occurred rarely. Therapy with metaxalone should be administered cautiously in patients with mild to moderate hepatic and/or renal impairment.
